What botox actually does

Botulinum toxin blocks the signal between nerve and muscle. The muscle stops contracting with the same force, the skin above it stops folding, and the line that folding created softens over the following two weeks. That is the whole mechanism.

Two consequences follow from it, and they matter more than any marketing claim:

• It works on lines made by movement — frown lines, forehead lines, crow's feet. Lines that are there when your face is completely still are structural, and toxin will soften them at best.
• It does not fill and it does not lift skin. If what bothers you is volume loss or sagging, toxin is the wrong tool. That is a different conversation and often a different treatment.

It is a prescription medicine and a medical act. In Colombia that means a physician, a product with INVIMA registration, and a dose decided after looking at your face in movement — not a price list applied blind.

Areas we treat, and what each one needs

Dose ranges below are the orienting figures from the literature for Botox®. They are a starting point, not a prescription: real dose depends on muscle strength, sex and goal.

What happens, and when

• Day 0. The session takes fifteen to twenty minutes. Small raised bumps at each injection point settle within the hour. You can go back to work.
• Days 3–5. First loss of movement. Usually asymmetric at this stage, which is normal and not a reason to panic.
• Day 14. Full effect. This is the day it is judged, and the day a top-up is decided if some point fell short.
• Months 3–4. Movement starts returning. Most people repeat between months four and six.
• Second and third year. With regular sessions the muscle partly atrophies from disuse. Many patients need fewer units and can space sessions further apart.

Who should not have it

Absolute contraindications: pregnancy and breastfeeding, neuromuscular disease such as myasthenia gravis or ALS, known allergy to any component, and active infection at the injection site.

Cases that need a conversation first: aminoglycoside antibiotics or muscle relaxants in progress, autoimmune disease in a flare, previous facial surgery that changed muscle anatomy, and very heavy eyelids or brows — where relaxing the forehead can make the hooding worse rather than better.

And one that is not a contraindication but is worth saying plainly: if you come in asking for botox because your face looks tired and the problem is volume loss or laxity, toxin will not fix it. We say so in the consultation rather than sell you a session.

How to choose where to have it done

Four questions. A clinic that answers all four can be compared with any other; one that does not is not cheaper — it is unmeasurable.

1. Which brand, and does it have INVIMA registration? INVIMA is Colombia's medicines regulator, the equivalent of the FDA. Ask to see the box.
2. How many units are included in that price? «One zone» is a container, not a dose. Two clinics can both quote «frown lines» and one apply 20 units and the other 8.
3. When was the vial reconstituted? Reconstituted toxin loses activity over days. A vial opened Monday and used Friday does not perform the same.
4. Who injects? It is a medical act. It should be a physician, and you should be able to find their credentials.
