The two families, and why the difference matters

Everything injected to add volume falls into one of two groups, and they work by different mechanisms:

• Hyaluronic acid. A gel that occupies space. The volume is there when you walk out. And — this is the decisive part — it is reversible: an enzyme, hyaluronidase, dissolves it within hours if you dislike the result or if a complication appears.
• Biostimulators (Sculptra®, Radiesse®). They do not fill: they leave a particle that triggers a controlled tissue response, and it is your own collagen that provides the volume over weeks. There is no enzyme to undo it.

Two rules follow, and they settle most decisions: in high-risk areas or in a first-time patient, hyaluronic acid wins because it is reversible; in diffuse loss of support across a hollowed mid-face, a biostimulator gives a more natural and longer-lasting result, in exchange for patience and irreversibility.

This practice works with biostimulators only: Sculptra® and Radiesse®. Hyaluronic acid is not offered, and that includes lips and cheeks. We say it up here rather than in the small print because it changes who this page is useful to.

If your assessment concludes that hyaluronic acid is what your case needs, you will be told so and referred. That is better than proposing what happens to be in stock.

The two products, compared

Neither is undone by an enzyme, and neither is used in lips. If what you want is immediate, reversible volume, that is hyaluronic acid and it is not offered here.

Between the two the choice is not about price: Radiesse gives volume the same day and is done in one session; Sculptra takes months and needs two or three, but lasts longer and the result is your own collagen.

The conversation almost nobody has: vascular risk

Filler has one serious, uncommon complication worth knowing about beforehand rather than after: vascular occlusion. If product enters an artery or compresses it, the territory that artery supplies loses its blood flow. It can end in skin necrosis and, in areas near the orbit, in vision loss.

It is uncommon, and it is why certain things are not negotiable:

• That hyaluronidase is in the clinic, that day. It is the antidote and its usefulness is measured in hours. Asking whether they have it is the single most useful question you can ask, and the one that most discomforts anyone who does not.
• That a physician injects, one who knows the vascular anatomy of the face.
• That the highest-risk areas — glabella, nose, tear trough and nasolabial folds — are treated with judgement and with a reversible product.

Signs that mean calling immediately: pain out of proportion during or after the injection, skin blanching, a purplish mottling, or any change in vision. None of these wait until morning.

Biostimulators are not quoted by millilitres but per session, and the figure above is the final price of the session. It is easier to compare than a hyaluronic acid filler, where a per-area price means nothing unless it states how many millilitres it includes.

How it works here:

• The quote is given in writing with the brand, the reference and the batch number before anything is applied. That document is what makes comparison possible.
• The number of sessions is set at the assessment, not over the phone. Radiesse usually takes one; Sculptra needs two or three, four to six weeks apart, each paid when applied.
