Why the cheek governs the rest of the face

The soft tissues of the face hang from the bony structures of the mid-face. When that support loses volume — through bone resorption and atrophy of the fat compartments — everything it held descends: the nasolabial fold deepens, the marionette line appears and the jawline loses definition.

Hence a rule that saves money and avoids odd results: in many cases, treating the cheek improves the fold without touching it. Filling the fold directly when the cause is higher up is what produces that heavy, wide mid-face everyone recognises as «filler».

Which product and in which plane

These areas need projection, not soft filling, and the product goes deep, resting on bone:

• Dense hyaluronic acid. The first option, especially for a first treatment, because it keeps reversibility.
• Radiesse®. Excellent in the chin and jaw angle, where firmness is needed, with the advantage that it also stimulates collagen. In exchange it does not dissolve, so the volume is decided more cautiously.
• Sculptra® makes sense when the loss is diffuse and no specific point needs projecting.

The chin deserves a note of its own: projecting it changes the profile and defines the jawline more than almost anyone expects, and it usually needs little product. It is one of the interventions with the best ratio of volume applied to change perceived.

What it costs

Filler is not quoted by «area» but by product and volume, and for a reason worth understanding before you compare: two people with the same complaint can need 1 ml and 4 ml. A closed per-area price that does not say how many millilitres it includes cannot be compared with anything — the same problem as botox units.
